Disability Community Vaccination Event – Hosted by Ability360

Disability Community Vaccination Event will be held for persons with disabilities, caregivers, children 5 years old and older,
and any community members living in the greater Phoenix metropolitan area.
Must be 5 years old or over. Minors must have an Authorized Health Representative for consent.
Vaccinations will be administered in a walk-in clinic setting at the Ability360 Sports & Fitness Center gymnasium.
ADA reasonable accommodations and American Sign Language (ASL) will be provided upon request in the sign-up system.
